By Submitting this form I understand that application is accepted, but team may not be accepted until payment is received by tournament convener for Southwest Minor Hockey Association.  Southwest Minor Hockey reserves right to decline applications at their discretion.

I accept responsibility for parents, players and staff of the team during tournament at tournament venues, will make parents aware of SMHA codes of conduct for parents and coaches and will observe arena and OMHA signage for responsible behavior.

I will assist tournament convener in removal of any participants from my team at the conveners or officials discretion.
